How to switch to Chippewa Valley Bank

STEP 1 – Open a Checking Account with Chippewa Valley Bank
Simply fill out the Account Information form and then visit your nearest CVB Office.  Once your CVB account has been established, stop using your former bank’s deposit account.  Make sure you leave sufficient funds in the account until all of your outstanding checks and automatic payments have cleared.
 
STEP 2 – Change your Direct Deposits and Automatic Payments
Print and complete the forms below and bring them to your nearest Chippewa Valley Bank office.  We will be glad to help you with your switch to CVB!

If you need assistance with any of these forms, our CVB staff would be more than happy to help!  Please bring a recent bill or statement, which includes an account number, for each deposit and/or automatic payment authorization to the nearest CVB office.
 
STEP 3 – Close your Former Bank Account
Print and complete the Authorization to Close Account Form and bring it to your new CVB office to be mailed.  We’d be happy to shred your former account checks and debit cards too!
